Exercise Rx
Complete all the steps outlined below and submit your Lab Report.

The Exercise Rx

In this Practicum, you will learn how to complete a Med-Fit Exercise Rx form.
Your volunteer Client is NOT needed for this Practicum, but you may want to practice explaining your completed Exercise Rx to your volunteer Client.
Needed Equipment & Materials;
  • Your client's completed Med-Fit Assessment Form (from Practicums 39, 41, 42)
  • Med-Fit Exercise Rx Form
  • Clipboard
  • Pen
  • Calculator
  • Therapeutic Exercise Rx Guidelines Chart
  • Score Interpretation & Follow-Up Timeframes Chart

1. Medical-Fitness Assessment - Test Results

Name & Date:
  • Refer to your clients Med-Fit Assessment form.
  • Print the client's full Name.​
  • Fill-in the Date of your client's Med-Fit Assessment
  • You should always issue your client's Exercise Rx on the same day of his/her Assessment.

Technician & Test Results:
  • Print your Name as the MFT.
  • Fill-in the client's Test Results. (Do not use %.)
  • Fill-in the client's total (aka: general) Med-Fit Score (Result).​

2. The F.I.T.T. Formula

For each of the 5 exercises listed in the left-hand column:
  • Refer to your client's Test Results.
  • Use the initial of each exercise (A, S, L, P, R) as a "superscript" on that Test Result's Fitness Level.
  • (A=Aerobic (Walk/Jog), S=Squats, L=Leg-Raises, P=Push-Ups, R=Inverted-Rows)
  • Underline the "mph" value or a "rep count" that corresponds to your Test Result for each exercise.
For the Walk/Jog aerobic exercise:
  • Circle the Speed (intensity) and Distance that you prescribe for your client's "Walk/Jog."​
  • This is usually the same as the intensity you underlined, but could be 1 higher or lower.
For each of the Calisthenic exercises:
  • Circle the Rep Count (intensity) that you prescribe for your client's Strength Training."
  • These are usually the same as the intensity you underlined, but could be 1 higher or lower.

3. Training Recommendations:

For your client's Training Recommendations:
  • Refer to the Grades that you assigned for the Strength Tests on your client's Med-Fit Assessment form:
  • Mark the exercises for which you recommend your client gets training for and/or modification of the exercises.
  • Encourage your client to follow-up regarding your recommended Therapeutic Fitness Training Sessions.

4. Exercise Logging Instructions:

  • Explain to your client that the table above is for logging his/her Rx Adherence for up to 12 weeks.
  • Instruct your client to "X" each "Exercise Day" that he/she did the prescribed exercises.
  • In the next Practicum (44), you will learn how to instruct your client in logging the specific details of each daily workout using a different form.

5. Progression:

  • Fill-in the # of weeks (1-4) that will qualify your client to increase the intensity of his/her exercise goals.
  • Explain to your client how he/she can refer to Part 2 of the Exercise Rx form to determine his/her next intensity level.

6. Follow-Up Recommendation:

  • Refer to your client's completed Med-Fit Assessment form.
  • Transcribe the Follow-Up entries onto the Exercise Rx form.​
8. Signature:
  • Print your Name & Phone # at the bottom of your client's Exercise Rx form
  • Write your Initials in the lower right corner of the form.
  • Encourage your client to contact you by Text if he/she has any questions about the Exercise Rx.
  • Remind your client to contact his/her personal physician if he/she has any issues related to his/her MEDICAL condition or treatment.
  • Example: Changes in home monitored blood pressure or blood sugar values related to medication dosages and increased physical activity.
